Children's Long Term Waiver
Coordinating Services for Children with Substantial Disabilities
We partner with the state and multiple Wisconsin counties to provide service coordination to families with children with substantial disabilities.
The Children's Long-Term Support Home and Community-Based Medicaid Waivers (CLTS Waivers) provide a structure in which Medicaid funding is available to support children who are living at home or in the community.
Types of Disabilities Covered in this Program
Children who quality for CLTS Waivers have substantial limitations in multiple daily activities as a result of one or more of the following disabilities:
- Developmental disabilities
- Severe emotional disturbances
- Physical disabilities
Wisconsin has three separate CLTS Waivers for each category of disability: developmental disabilities, severe emotional disabilities and physical disabilities.
Some children qualify for more than one waiver. The family and their service coordinator will determine which single waiver is the most appropriate source of funding for the child. Funding through this program can be used to support a range of services based on an individual assessment of the child and his or her needs.
