Community-Based Residential Facilities

To address mental health crisis needs in Wisconsin communities, we own and operate multiple Community-Based Residential Facilities (CBRF) in various communities

across Wisconsin to serve individuals who are referred by the county of their residence. We build programs and staff them with trained professionals who serve residents and address their specific needs as identified by the county. Our programs are CCS-reimbursable by the state when the primary diagnosis is mental health. In addition, these programs meet all state regulatory licensing requirements.

Depending on the program, our staff members either provide 24/7 staffing or day and evening supervision. Staff members receive training to match the specific types of services the residents require. Services include: medication monitoring, group therapy, and independent-living and coping skills. In addition, we train our staff in trauma-informed care with a focus on recovery. Our staff also work to maintain the residents’ medical and mental health and suicide prevention at all residences. However, professionals at our facilities do not provide personal care for residents, so residents need to be fully ambulatory and physically capable to reside in our residential units.

Length of stay at our residential facilities ranges from a two-week crisis program to many years, and the length of stay is customizable to meet the needs of those in our care. Our houses offer home-like environments with a maximum of two people per room, and some residents have private rooms.

Individuals in our residential mental health and crisis programs reside in their home communities to make connections and find local resources – to serve them now and in the future. The goal of the program is for residents to resume their best lives possible while providing the least-restrictive setting possible, always with a recovery focus.

Veteran Housing and Recovery Services

LSS is proud to partner with the Wisconsin Department of Veterans Affairs to provide housing and services to veterans in three locations across Wisconsin.  We are thankful for their service to our country, their sacrifices and are honored to serve these veterans now.

Information and referral is available 24 hours a day, seven days a week – throughout the year and in more than 90 languages.

Crisis Facilities

Eastwood 

Eastwood Crisis Facility is an eight bed licensed Community Based Residential Facility (CBRF) located in Outagamie Co. This program serves ambulatory adult men and women with mental health and/or AODA issues who are experiencing a crisis from any of life’s difficult circumstances, such as loss of housing, medication changes, suicidal ideation, and grief or trauma. Individuals are referred to Eastwood Crisis through Outagamie Co. referrals. A typical stay at Eastwood Crisis Facility is less than two weeks. Our skilled Mental Health Specialists provide 24 hour supervision and supportive services such as:

  • Medication Administration and Education
  • Health and Wellness Monitoring
  • Independent Living Skills
  • Interactive Groups
  • Transition Services and Discharge Planning

Eastwood is in a residential neighborhood and offers a home-like environment to achieve safety and stabilization in the least restrictive setting possible.

 

Residential Facilities

Grandstone CBRF – Appleton, WI 

Grandstone is a licensed Community Based Residential Facility (CBRF) located in Appleton, WI. This program provides 24 hour supervision to ambulatory adult men and women diagnosed with mental illness. Referrals to Grandstone are made through County and Managed Care Organizations. A typical stay at Grandstone depends on the needs of the individual and can range from six months to a couple of years. Our skilled Mental Health Specialists provide supportive services such as:

  • Individualized Service Planning
  • Medication Administration and Education
  • Assistance with managing medical needs and appointments
  • Health and Wellness Monitoring
  • Independent Living Skills both 1:1 and in group setting
  • Behavior Management
  • Assistance with employment readiness and search
  • Assistance with housing resources
  • Ongoing opportunities for leisure and community involvement
  • Transportation planning assistance for vocational or education needs
  • Transition services and discharge planning

Grandstone program offers a home-like environment in a quiet neighborhood, near a city bus route for access to local businesses for shopping and vocational needs to provide the individual resources to live quality life while receiving services towards independence.

 

Preble CBRF – Green Bay, WI 

Preble is a licensed Community Based Residential Facility (CBRF) located in Green Bay, WI. This program provides 24 hour supervision to ambulatory adult men and women diagnosed with mental illness. Referrals to Preble are made through County and Managed Care Organizations. A typical stay at Preble depends on the needs of the individual and can range from six months to a couple of years. Our skilled Mental Health Specialists provide supportive services such as:
