Adult Day Centers
Our adult day programs serve physically and/or cognitively challenged adults at a variety of locations in Wisconsin. The programs provide respite for caregivers, and help individuals remain living in their own homes by offering services that support independence.
Senior Connection & Caregiver Support
Our Senior Connection program connects older adults throughout the U.S. with valuable resources and support systems. Our Senior Connection staff provide answers and assistance to help seniors and families find the best solutions for specific challenges, including caregiver support and education, home-delivered meals, and more.
Memory Loss Program
Our memory enhancement program, called MindWorks, uses current research and successful outcomes from other memory care programs to help anyone experiencing early memory loss. Guided by an evidenced‐based curriculum, MindWorks provides a proactive approach to improving memory function, exercising the brain through various cognitive, creative and physical activities.

Senior housing options are available across the two states, residents enjoy a range of amenities, from underground parking to activity coordination. Learn more about our properties.
Senior Transportation Services
We offer a centralized transportation service called Making the Ride Happen, available to seniors in Outagamie, Calumet and Winnebago counties. Making the Ride Happen helps seniors with disabilities, or those who simply have trouble getting around, get to medical appointments, grocery shopping, day programs, hair salons, banks and pharmacies. Transportation options include public bus, paratransit, rural and meal site transportation, or volunteer driver programs.
Connections, an IRIS (Include, Respect, I Self-Direct) consultant agency by Lutheran Social Services (LSS), serves IRIS participants who are determined to be eligible by the Aging and Disability Resource Center (ADRC) to participate in the IRIS program in Wisconsin. Experts at the ADRC will establish your functional, financial and non-financial eligibility for Wisconsin's long-term care programs.
Health and Wellness Services - Appleton
Our health and wellness clinic provides services, at no cost or reduced rates, in Appleton for seniors and others in the community in need of care. Wellness services include basic medical care and advice that's provided by a registered nurse as well as specialty health services such as our foot care clinic, seasonal flu shots, lab screenings, and support groups for visually impaired persons.
