Individualized Community Supports
Live at Home
We offer individualized community supports to help you live in your home successfully if you live with disabilities or mental illness, or are an older adult and meet qualifications for this supportive home care.
After our staff has received a referral, we provide an assessment to determine what daily living skills you need, how we can assist you or whether we can help you to learn a skill that you can hand with ease in the future.
At-home assistance may be cooking, cleaning, laundry or other services that make it possible for you to live in your home. Our assessment also includes how often you will need services to help you stay in your home. We will develop an individual plan that works for each person we assist. As our client of supported home care, we may help support you for a few months or many years.
To qualify for individualized community supports, you need to be 18 years old, have a disability or a mental illness, or are an older adult who either qualifies for Social Security or Disability Insurance. Most clients are referred from the IRIS program or a managed care organization. We do accept private pay for these services as well.
