Self-Directed Care and Support
We offer community-supported living services to assist you in self-directed services and support in the IRIS (Include, Respect, I Self-Direct) program and through managed care organizations.
Once we receive a referral from one of these organizations, one of our employees specializing in self-directed community services will meet with you in your home, or a place you would like, to work with you to determine your support needs and how we can help you achieve them.
The goal of our services is to help you become more independent so you can live the life you choose as a person with disabilities or an older adult. We work as project managers and follow through with your plan – offering you flexibility and continual assessment as we move forward to help you reach your goals.
Understand and manage services
To get started, we will meet with you to identify goals, establish milestones, research available staff and services, help you make payments for staff and services, and follow up with you throughout our service.
We will help you create a plan for your services and support. Our focus is on building a relationship with you and to help you understand the often-complex IRIS and managed care organization agreements for your services and support. Our staff can also help you manage your employees, like nurses, to better serve your needs. In addition, our staff will match your needs with services available in your community.
Living Independently With a Disability
Cole is in his 30s and he’s living on his own in for the very first time. He takes the bus, has a job and leads a full and active life in Eau Claire, Wisconsin. His mom is so proud and happy that Cole was given the chance to succeed living on his own. She said he loved it from the get go!
