While disabilities present unique challenges, most people can live successfully in their own communities. Our staff members help more than 2,000 people with disability services, providing custom levels of support, including support for older adults who also need these services. This support can range from a few hours per week to 24 hours per day, seven days a week – year round.
Individualized Community Supports
We offer individualized community supports to help individuals with disabilities live at home successfully. Our staff members provide an assessment to determine what daily living skills you need, how we can assist you or whether we can help you to learn a skill that you can hand with ease in the future.
Meaningful Day Through Community Connections
Meaningful Day Through Community Connections services offers people with disabilities the opportunity to fill their days with meaningful activities.These programs include exploring employment opportunities, volunteering, attending community education classes or post-secondary education, or establishing community connections that may have been lost or that are new to your life.
Our Housing Options
We offer housing services and options that meet the unique needs of anyone living with disabilities. Services range from on-site service coordinators to more personalized help aimed at keeping individuals living in their own homes independently.
Community Supported Living
We offer community-supported living services focused on self-directed services and support in the IRIS (Include, Respect, I Self-Direct) program and through managed care organizations. Our employees specialize in self-directed community services and work individuals to determine specific needs and how we can help you achieve them.
Connections, an IRIS (Include, Respect, I Self-Direct) consultant agency by Lutheran Social Services (LSS), serves IRIS participants who are determined to be eligible by the Aging and Disability Resource Center (ADRC) to participate in the IRIS program in Wisconsin. Experts at the ADRC will establish your functional, financial and non-financial eligibility for Wisconsin's long-term care programs.
Adult Day Centers
If you have a disability, our adult day centers offer community-based and in-center supports that help you remain independent and living in your own home.
Support Broker Services
Support brokers help to coordinate IRIS services or other self-directed services you receive through a managed care organization. We work together with you and your family, finding providers in the complex network of resources and then managing those relationships, relieving the burden on you and your family.
