LSS team members provide skill-building as a subcontractor of a certified county, tribe, or managed care organization, and in alignment with an individual’s service plan.

We work collaboratively with other providers and referents to provide services that help people live healthier, more productive lives.

People experience a variety of symptoms, needs, or diagnosis can benefit from the skill-building services noted.

Skill Building

Skill building and/or training services are provided across the state and through many programs.  Services include:

  • Recovery support including emotional, informational, instrumental, and affiliated support.
  • Education and information resources about the consumer’s mental health and/or substance abuse issues.
  • Guidance about managing and coping with mental health and/or substance abuse issues.
  • Social and emotional support for dealing with mental health and/or substance abuse issues.
  • Assistance in increasing engagement in treatment, developing appropriate coping strategies, and providing aftercare and assertive continuing care.
  • Relapse prevention support and periodic follow-ups designed to provide less intensive services as the consumer progresses in recovery.
  • Medication management (non-prescriber) services include education regarding medications, assistance with compliance, and ongoing symptom monitoring.
  • Parenting skills include spending time with the family either in the home or a home-like setting to teach caregivers critical parenting skills.

Skill training may be provided individually or in a group setting.  Skill training may be provided by various methods, including but not limited to:

In-Home Recovery Services

In-Home Recovery Services provides mental health supports to individuals and independent-living-skills training offered within their own homes and communities. Support and services are individualized to meet the consumer’s needs and dreams so they can reach their desired independent-living goals. Services are recovery-oriented.

Family Partnerships Initiative (FPI) and Youth Enterprise for Success (YES)

We provide wraparound services to at-risk children and their families, particularly those who have struggled with traditional service provision and are at-risk for out-of-home placement or working toward reunification. Using a family-centered and community-based philosophy, we focus on maintaining or reuniting children with their family by creating individualized care plans. Services focus on the child’s well-being and progress, but often incorporate other family members and supports. The program is most beneficial when the family is open to receiving services and participating in their child’s treatment planning.

FPI and YES help the family live more successfully in the community, so they ultimately need fewer formal services and interventions.

The program provides an array of services beyond traditional financial and geographic boundaries to develop a creative and flexible continuum of care.

FPI and YES care coordinators are assigned a lower caseload due to the intensive nature of services so they can provide thorough and focused support and care.

Referrals are made by the county, Families complete an assessment process to determine eligibility.

FPI services last twelve months but can extend beyond that based on the family’s needs.

YES services have no time restrictions

FPI is Funded through the county or CLTS

YES is funded through the referring county, CLTS or CCS.

Community Supported Living (CSL)

Community Supported Living is an individualized, innovative approach to the delivery of comprehensive services, offering supports that fit a person’s unique lifestyle and level of independence.  CSL provides a diverse range of supports and offers individualized scheduling.

Services can include:

  • Money management
  • Meal preparation
  • Household care, maintenance and laundry
  • Shopping for food and other necessities
  • Community involvement
  • Medical care management

Support Broker

Support Brokers provide supports for adults who are self-directing their long term care services and require additional assistance with planning, securing and directing some part of those services in order to live and maintain an independent life of their choosing.  Support Brokers can assist the individual to be a successful employer by working together on developing employee job recruitment, interviewing, scheduling, creating work policies, reviewing timecards and assistance with on-going employee support.

  • Act as a liaison with professionals
  • Assist with paperwork completion
  • Connect people with community resources
  • Assist to find housing

Crisis Services

Support for Mental Health and Substance-Abuse Challenges

We offer residential and crisis services, available through a county referral, to assist individuals who are experiencing a mental health crisis and who are in need of stability and the skills to live independently in the community.
