Beneficiary Designations
Your support for the neighbors served by LSS can continue even beyond your lifetime when you name LSS or the LSS Foundation as a primary or secondary beneficiary of an asset such as your retirement plan, insurance policy, commercial annuity, or donor advised fund. The process is quick and easy. It typically requires nothing more than completing a simple form provided by your plan or policy administrator. It costs you nothing now, and it’s a planned gift that gives you more security for the future – your plan or policy will be there to support you until you or loved ones no longer need it.
A further benefit is being able to maximize how much of your estate actually reaches the people, places, ministries, and charities that mean the most to you. Many donors choose to leave assets including retirement plans, insurance policies, commercial annuities, and U.S. savings bonds to tax-exempt charities like LSS and the LSS Foundation because the full amount of the gift will pass to the charity. Choosing to pass less tax-burdened assets such as real estate and other property on to heirs can maximize the amount of your estate that reaches your intended beneficiaries.
By naming the LSS Foundation as a beneficiary, you’ll continue to provide help to neighbors served by LSS for generations to come. Or, you may choose to name Lutheran Social Services of Wisconsin and Upper Michigan in order to direct your gift to LSS programs for immediate use when received.
It’s an easy process you can do today. Here are the steps to take:
- Request a change-of-beneficiary form by contacting the administrator of your retirement plan, insurance policy or bank account to, or simply download a form from your provider’s website.
- Decide what percentage (1 to 100) you would like LSS or the LSS Foundation, Inc. to receive and name us, along with the percentage you chose, on the beneficiary form. Your form may require the following information:

Please consider completing our Planned Gift Statement of Intent Form and sending us a copy. This helps to ensure our records are up to date and helps us make sure we can put your gift to use according to your wishes.
