Case Management

LSS provides case management services for individuals who have developmental and physical disabilities, and/or mental health issues. Case managers advocate for the individual’s health, care, happiness and quality of life.

LSS believes that the best-case management is provided through a team approach, with the person being served as both a member and focal point of the team. Service providers, the case manager, guardians and other involved support people make up the remainder of the team. It is the case manager’s responsibility to listen, investigate, obtain information and arrange for services. All team members must feel that their ideas are heard and action taken is in the best interest of the individual being served. LSS believes the dignity of people with disabilities is enhanced when they exercise control over their own lives.
